Deer Oaks Behavioral Health — Long-Term Care Clinical Services is seeking a Psychiatric Mental Health Nurse Practitioner to provide in-person assessments and medication management for residents in local facilities. The role emphasizes Medicare-compliant gradual dose reduction and geriatric-focused care within an interdisciplinary team.

Qualifications

• Active, unrestricted PMHNP or PMHNP-BC license.
• Valid DEA registration.
• Strong clinical judgment working in LTC environments.

Responsibilities

• Conduct comprehensive psychiatric evaluations and mental health assessments for LTC residents.
• Develop, implement, and adjust individualized treatment plans.
• Prescribe and manage psychotropic medications per Medicare and LTC best practices.
• Implement Medicare-compliant gradual dose reduction (GDR) strategies.
• Monitor and treat chronic mental health conditions.
• Collaborate with interdisciplinary teams—nursing, medical providers, social services, rehabilitation, and facility leadership.
• Complete accurate documentation in EMR.
