Job summary

SSM Health Monroe Hospital in Monroe, WI, seeks a Board-Certified Physician to join a geriatric medicine team serving skilled nursing facilities. You set your weekly schedule, with Monday–Friday hours and no on-call or weekend obligations, while providing high-quality elder care.

Responsibilities include routine assessments, medications, care plans, and coordination with a team of Geriatric Nurse Practitioners. EPIC documentation and interdisciplinary meetings are part of the role.

Qualifications

• Completion of an ACGME Accredited Residency Program.
• Board Certification in Family Medicine OR Internal Medicine.
• Dual WI and IL licensing.
• Active Illinois medical license before start date.
• DEA certification.
• Current BLS certification.

Responsibilities

• Provide comprehensive medical care to residents in skilled nursing facilities.
• Perform routine assessments, medication management, and care planning.
• Respond to acute changes during scheduled hours.
• Supervise Geriatric Nurse Practitioners and collaborate with the team.
• Document in EPIC EMR and participate in interdisciplinary care team meetings.
• Communicate plans with patients, families, and facility staff regarding treatment plans.
