Job summary

Gary and Mary West PACE in North County is seeking a Physician Assistant to provide assessment, primary care, and continuous medical coverage for PACE participants within an interdisciplinary team, following GMWP standards. Under the supervision of the Medical Director, you will collaborate with PACE Physicians to develop care plans and deliver high-quality care in the center and in participants' homes.

• PA degree required.
• Active PA license required.
• DEA license required.
• Prescriptive authority certification required.
• BLS/CPR certification current.
• Driver's license with insurance.

Responsibilities

• Provide primary care and acute illness evaluation for participants with PACE physicians in center and homes.
• Perform comprehensive in-person assessments (initial and semi-annual) and coordinate a care plan with the IDT.
• Collaborate with PACE physicians on discharge planning from facilities.
• Refer participants to medical specialists as indicated.
• Manage care in nursing homes with regular visits per standards.
• Participate in rotating night and weekend calls.
• Review outcomes/utilization data with Medical Director to improve policy and reduce unnecessary care.
• Protect privacy and follow company policies; maintain confidentiality.
