Key Responsibilities
  • Provide comprehensive primary care to elderly residents in long-term care and skilled nursing facilities.
  • Conduct initial assessments and routine follow-ups, addressing acute and chronic conditions.
  • Lead interdisciplinary care teams, including nurses, therapists, case managers, and social workers.
  • Develop and monitor individualized care plans to maximize quality of life and function.
