Qualifications

• Must possess a current Physical Therapist license issued in New York State.
• Bachelor's Degree in Physical Therapy; Master's or Doctorate preferred.
• Strong communication and interpersonal skills for training patients and collaborating with team members.
• Ability to organize and prioritize patient care in a fast-paced rehab environment.

Responsibilities

• Conduct thorough assessments of residents to evaluate mobility and therapy needs.
• Design and implement individualized treatment plans to improve strength, mobility, and functional independence.
• Deliver therapeutic exercises, manual therapy, and modalities as needed.
• Monitor and document residents' progress per protocols; adjust treatment strategies accordingly.
• Collaborate with interdisciplinary team to develop comprehensive care plans.
• Educate residents and families about home exercise programs and health maintenance.
• Maintain compliance with state and federal regulations and facility policies.
