Job summary

Sayville Rehabilitation & Nursing Center is seeking an experienced Physical Therapist (PT) to evaluate and treat residents under physician direction. You will develop individualized plans, document progress, and educate patients and families to promote best outcomes.

The ideal candidate has licensure, strong documentation skills, and experience in long-term/geriatric care, with a collaborative approach across disciplines.

Qualifications

• Graduated from an accredited Physical Therapy program/university.
• Valid professional license and registration for the state of practice.
• Experience supervising Physical Therapy Assistants.
• Long-term care/geriatric care experience preferred.
• Strong documentation skills and record-keeping.
• Familiarity with HIPAA and patient confidentiality.

Responsibilities

• Assess referred patients and provide direct PT care to determine rehabilitation needs.
• Establish and administer treatment programs with goals aligned to patient capacity.
• Document patient condition, progress, and outcomes; educate patients and families.
• Collaborate with all disciplines to deliver patient-centered care.
• Supervise and guide Physical Therapy Assistants as needed.
• Maintain professionalism and adhere to scheduled hours; support Rehabilitation Director.
