Job summary

Sans Souci Rehabilitation & Nursing Center is seeking an experienced Physical Therapist for our facility in New York. The PT will assess patients, develop and administer treatment plans, and document progress while educating patients and families.

Must supervise PT Assistants and ensure compliance with confidentiality and residents' rights. Geriatric/long-term care experience preferred; strong documentation skills required; PPS scheduling knowledge a plus.

Qualifications

• Graduate from an accredited Physical Therapy program.
• Valid license and registration for state of practice.
• Experience supervising Physical Therapy Assistants.
• Experience in long-term care and/or geriatric care preferred.
• Strong documentation skills.
• Experience with case mix index and PPS scheduling preferred.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals aligned to patient capacity.
• Document patient condition and progress; educate patients and families in PT methods.
• Maintain confidentiality and adhere to HIPAA and Residents' Rights.
• Collaborate with all disciplines to deliver patient-centered care.
