Job summary

Quality Center for Rehabilitation and Healing in Hendersonville, TN seeks an experienced Physical Therapist (PT) to assess and treat rehab patients in a long-term care setting. You will develop individualized treatment plans, document patient progress, and collaborate with the care team to achieve optimal outcomes.

The role supervises Physical Therapy Assistants, maintains HIPAA compliance, and delivers high-quality patient care with professionalism. Daily pay available and competitive benefits.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Oversee and supervise Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care preferred.
• Well versed with documentation writing.
• Perform evaluations, weekly progress notes, recertification and discharge documentation.
• Collaborates with all disciplines to provide patient-centered care.
• Demonstrate exceptional patient service and clinical excellence.
• Punctuality, professionalism, and adherence to scheduled hours.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals based on patient capacity.
• Document patient condition and educate patients and families in PT methods.
• Supervise Physical Therapy Assistants and coordinate with care team.
• Maintain HIPAA compliance and complete required documentation.
