Job summary

Therapy Management Corporation (TMC) is seeking a licensed Physical Therapist to provide patient-centered care to adults and older adults in a collaborative clinical setting. You will evaluate, plan, and implement PT programs aimed at improving mobility, independence and quality of life.

Qualifications

• Licensed PT in the state of practice or license eligibility.
• Graduate of an accredited Physical Therapist program.
• Interest or experience in skilled nursing, long-term care or geriatric rehabilitation.
• Passion for patient-centered care and improving quality of life for older adults.

Responsibilities

• Evaluate patients and provide personalized physical therapy treatments.
• Develop and implement patient-centered treatment plans for long‑term goals.
• Deliver hands-on therapy interventions to support recovery and functional independence in older adults.
• Help patients restore functional mobility and safely perform daily activities.
• Partner with interdisciplinary teams to drive outcomes and continuity of care.
• Track progress and maintain accurate clinical documentation.
