Citrus Health and Rehabilitation in Inverness, FL is seeking a Physical Therapist to evaluate and treat residents in a skilled nursing facility. You will collaborate with an interdisciplinary team to improve mobility, function, and quality of life while adhering to regulatory standards.

The role requires a DPT and Florida PT license; CPR/BLS is preferred. A $10,000 sign-on bonus is offered for full-time clinicians, with competitive wages and benefits.

Formación

• DPT from an accredited physical therapy program.
• Florida Physical Therapist License in good standing.
• CPR/BLS certification preferred; Florida licensure is required for practicing PTs in the state.

Responsabilidades

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
• Assess resident mobility, strength, balance, gait, range of motion, pain levels, and functional abilities.
• Provide skilled therapeutic interventions, including therapeutic exercise, gait training, balance training, transfer training, neuromuscular re-education, and manual therapy.
• Establish realistic short-term and long-term rehabilitation goals.
• Monitor resident progress and modify treatment plans as appropriate.
• Educate residents, families, and caregivers regarding treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with physicians, nursing staff, occupational therapists, speech-language pathologists, and other healthcare professionals to coordinate resident care.
• Complete accurate and timely documentation in accordance with Medicare, Medicaid, Florida regulations, and facility policies.
• Participate in care plan meetings and interdisciplinary team conferences.
• Ensure compliance with infection control, safety, and quality assurance standards.
• Supervise PTAs and rehab aides as permitted by state regulations.
• Maintain productivity standards and support facility performance goals.
