Job summary

Element Care seeks a Personal Care Specialist serving on the PACE Interdisciplinary Team. Under a licensed nurse, you will plan, implement, and evaluate personal care and safety for participants at the ADH and in the community.

You will assist with personal care, meals, vital signs, and transfers, while reporting changes to the nurse and escorting participants to appointments. A High School diploma or equivalent and HHA certification are preferred.

Qualifications

• Experience working with geriatric populations.
• Strong verbal and written communication abilities.
• Ability to work well within a team.
• Ability to perform vital signs and assist with transfers.

Responsibilities

• Assist participants with personal care at the site and in the community.
• Assist with meals and 1:1 feedings, following diet restrictions.
• Monitor vital signs and assist with transfers.
• Report changes to the primary nurse and coordinate with the care team.
• Escort participants to appointments and restock supplies.
• Maintain cleanliness of exam rooms and related areas.
• Attend trainings and adhere to policies and procedures.
