Job summary

Responsibilities

• Education - Participates in identifying and meeting learning needs of self.
• Evaluation of Care - Identifies expected patient outcomes and collaborates with interdisciplinary team.
• Implementation - Delivers care for patients with complex needs and coordinates care per policy.
• Leadership - Supports unit goals and interdepartmental collaboration.
• Patient Assessment - Synthesizes assessment data and plans discharge needs.
• Planning - Collaborates with patient/family to plan care and use multidisciplinary resources.
• Research - Reads journals and brings ideas to staff for evaluation.
