1 Care Partners is seeking a compassionate Physician Assistant (PA) to provide high-quality healthcare services in a Skilled Nursing Facility in Harrisonburg, VA. You will collaborate with physicians, nursing staff and other professionals to assess, diagnose and treat residents, focusing on geriatric care and continuity of care.

Qualifications

• Must hold an Active Virginia PA license.
• Experience in Skilled Nursing Facility or geriatric setting preferred.
• Strong clinical assessment and decision-making skills.
• Ability to work independently while coordinating with a multidisciplinary team.

Responsabilités

• Conduct comprehensive patient assessments and develop individualized treatment plans.
• Diagnose and manage acute and chronic conditions in coordination with physicians.
• Educate patients, families, and facility staff on disease management and preventive care.
• Maintain accurate and timely documentation in compliance with regulatory guidelines.
• Collaborate with interdisciplinary teams to ensure quality patient outcomes.
