Job summary

Bayberry Commons in Rhode Island seeks a Certified Occupational Therapist to evaluate, plan, and implement individualized treatment programs for residents in a skilled nursing environment. Qualifications

• Bachelor’s, Master’s, or Doctorate degree in Occupational Therapy from an accredited program.
• Current Rhode Island licensure (or eligibility) as an Occupational Therapist.
• Prior experience in a skilled nursing facility or geriatric setting preferred.
• Strong communication, documentation, and interpersonal skills.
• Commitment to resident-centered care and rehabilitation excellence.

Responsibilities

• Evaluate residents’ functional abilities and develop individualized treatment plans based on clinical findings and goals.
• Provide direct occupational therapy services including therapeutic exercises, ADL training, and adaptive equipment recommendations.
• Document all evaluations, treatments, and resident progress in accordance with professional and regulatory standards.
• Collaborate with nursing, physical therapy, speech therapy, and other disciplines to ensure coordinated, comprehensive resident care.
• Educate residents, families, and caregivers on techniques to improve independence and safety.
• Participate in care plan meetings and discharge planning as needed.
• Maintain compliance with all applicable state, federal, and facility policies and procedures.
• Supervise Certified Occupational Therapy Assistants (COTAs) as required.
