Job summary

Greenhills Health and Rehabilitation Center is seeking an Occupational Therapist to join our team. You will assess referred patients, develop and supervise rehab plans, and work under the direction of the Physician.

You will educate patients and families, document progress, collaborate with interdisciplinary team, and supervise OT assistants. Prior long-term or geriatric care experience preferred; must maintain confidentiality and professional standards.

Qualifications

• Must have graduated from an accredited OT program.
• Must hold a valid OT license and state registration.
• Experience supervising OT assistants.
• Experience in long-term or geriatric care preferred.
• Strong documentation and discharge planning experience.
• Excellent patient service and professional conduct.
• HIPAA and patient confidentiality compliance.

Responsibilities

• Assess referred patients and provide direct patient care.
• Develop and administer individualized treatment plans.
• Set goals based on patient capacity and tolerance.
• Document patient condition and progress weekly.
• Educate patients and families on therapy methods.
• Collaborate with other disciplines to provide patient-centered care.
• Supervise OT assistants.
