Job summary

Renewal Rehab seeks a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. In this onsite role, you will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You’ll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Licensed OT for geriatrics in SNF/post-acute settings.
• Documentation in EMR and collaboration with care teams.
• Education of families and caregivers regarding care plans.
