Renewal Rehab seeks a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role involves evaluating residents, creating individualized treatment plans, and delivering evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers to support daily functional outcomes and quality of life.

Qualifications

• Must be a licensed Occupational Therapist.
• Evaluate residents and develop individualized treatment plans.
• Deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.
