Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. In this onsite role, you will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Licensed OT providing care to seniors in skilled nursing facilities.
• Experience with post-acute populations is preferred.
• Ability to develop individualized treatment plans.

Responsibilities
