Job summary

Teamwork Therapies in San Mateo, CA is seeking a licensed Occupational Therapist to provide rehabilitative services in a Skilled Nursing Facility. You will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve independence and safety.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current California Occupational Therapist license in good standing.
• Current CPR/BLS certification.
• Excellent written and verbal communication skills.
• Ability to work independently and collaboratively within an interdisciplinary team.

Responsibilities

• Perform comprehensive occupational therapy evaluations for residents in an SNF.
• Develop and update individualized treatment plans with measurable goals.
• Provide evidence-based OT interventions for ADL, motor, and cognitive goals.
• Document evaluations, progress notes, discharge summaries, and recertifications.
• Collaborate with physicians, nurses, PTs, SLPs, social workers, and dietitians.
• Educate residents and families on home programs, safety, and equipment.
