Job summary

Chatham Hills Subacute Care Center in Hillside, NJ is seeking a dedicated Occupational Therapist to join our rehabilitation team. The role includes patient assessments, developing treatment plans, and guiding families through recovery.

You will supervise OT Assistants, document care, and collaborate with all disciplines to deliver patient-centered outcomes. A $5,000 sign-on bonus is offered and HIPAA compliance is required.

Qualifications

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals based on patient capacity.
• Document condition and educate patients and families in therapy methods.
