Job summary

Chatham Hills Subacute Care Center in New Jersey is seeking an Occupational Therapist to join our rehabilitation team. You will assess patients referred for therapy, design individualized treatment plans, and monitor progress.

Under the direction of a physician, you will set achievable goals, document patient status, educate families, and supervise OT Assistants while collaborating with all disciplines to deliver patient-centered care.

Qualifications

• OT degree from an accredited program and a valid state license.
• Experience in long-term or geriatric care preferred.
• Strong documentation and record-keeping skills.
• Ability to collaborate with a multidisciplinary team.

Responsibilities

• Assess referred patients to determine rehabilitation needs.
• Develop and administer treatment programs with specific goals.
• Educate patients and families on therapy approaches.
• Supervise Occupational Therapy Assistants.
• Maintain documentation including evaluations, progress notes, and discharge summaries.
• Ensure confidentiality and comply with HIPAA and Residents' Rights.
