Job summary

Renewal Rehab seeks an Occupational Therapist to provide patient-centered care to seniors in skilled nursing and post-acute settings. You will assess functional abilities, create evidence-based plans, and deliver interventions that promote independence, safety, and quality of life.

Collaborating with an interdisciplinary team, you will maintain thorough documentation and educate patients, families, and staff.

Qualifications

• Assess functional abilities and gait, balance, and ADLs to inform therapy.
• Develop and implement evidence-based treatment plans tailored to seniors in skilled nursing and post-acute care.
• Deliver individualized interventions to promote independence, mobility, and safety.
• Coordinate care with an interdisciplinary team to optimize outcomes.
• Maintain accurate, timely documentation and follow regulatory standards.
• Educate patients, families, and staff to support home and facility goals.
