Renewal Rehab is seeking an Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute care settings. You will assess functional abilities, create evidence-based treatment plans, and deliver interventions that promote independence, safety, and quality of life.

You’ll collaborate with an interdisciplinary team, maintain accurate documentation, and educate patients, families, and staff.

Responsibilities

• Deliver individualized occupational therapy interventions to promote independence, safety, and quality of life.
• Collaborate with an interdisciplinary team to optimize patient outcomes.
