Job summary

Providence PACE in Hillsboro is seeking an Occupational Therapist to join the Interdisciplinary Team at the Tanasbourne center. You will perform OT evaluations and develop and implement the OT plan of care in accordance with PACE policies, coordinating care with providers, caregivers and family members.

This full-time, on-site role offers a $5,000 hiring bonus for eligible external candidates and a comprehensive benefits package designed to support you and the people you serve in the community.

Qualifications

• Bachelor's Degree in Occupational Therapy required.
• Oregon OT license upon hire.
• Within 30 days: National Provider BLS – American Heart Association.

Responsibilities

• Perform OT evaluations and develop the OT plan of care.
• Implement OT plan of care per PACE policies and coordinate with the Interdisciplinary Team and external providers.
