Elderwood at Cheektowaga seeks a dedicated Occupational Therapist to deliver skilled therapy and improve residents’ independence. You’ll work within a care-focused, interdisciplinary team, ensuring high-quality, person-centered services in a supportive environment.

Qualifications

• Active, state-specific Occupational Therapist license (required).
• Commitment to high-quality, resident-centered care.
• Strong communication and interdisciplinary collaboration skills.
• Passion for helping individuals regain independence in daily activities.
• Ability to communicate well verbally and in writing.
• Sense of responsibility and ability to work well with residents, families, and other staff.

Responsibilities

• Initiates skilled occupational therapy treatments per physician orders.
• Maintains evaluations, progress notes, MDS records and discharge summaries.
• Confers with health care staff to enhance rehabilitation.
• Provides input for Care Planning meetings and covers during absences.
• Maintains statistical records of treatments.
• Assists with in-service training and education.
• Records and updates care plans.
• Submits requisitions for supplies.
