Job summary

Qualifications

• Advanced degree in Occupational Therapy.
• Experience in interdisciplinary teams preferred.
• Valid driver's license and immunizations up-to-date.

Responsibilities

• Assess participants' functional abilities and home environments.
• Develop and update individualized treatment plans with IDT.
• Deliver therapy across centers, clinics, homes, and contracted facilities.
• Educate participants and caregivers on ADLs and safety.
• Supervise and mentor COTAs and train staff on safe practices.
• Maintain compliant documentation and participate in IDT meetings.
