Job summary

Senior LIFE PACE in York, PA is seeking a licensed Occupational Therapist for a full-time role providing comprehensive OT services to seniors in a home- and community-based setting.

Responsibilities include developing treatment plans, documenting visits, assessing home environments, and collaborating with interdisciplinary teams to expand and enhance the Plan of Care. PA licensure and at least one year of geriatric OT experience are required.

Qualifications

• Must be a licensed Occupational Therapist in Pennsylvania.
• Minimum of 1 year experience working with elderly populations.
• Ability to develop and implement comprehensive Plans of Care.
• Strong documentation skills and ability to collaborate with interdisciplinary teams.

Responsibilities

• Develop occupational therapy treatment plans for new and existing members based on assessments.
• Document visits and changes to member status to ensure cohesive care across departments.
• Assess home environments to minimize hazards and provide in-home support as needed.
• Educate LIFE Center staff and member families on OT plans and ensure execution.
• Collaborate with interdisciplinary teams and communicate changes across care teams.
• Coordinate with dietary staff on adaptive feeding techniques as needed.
