Job summary

BoldAge PACE is seeking an Occupational Therapist to join our interdisciplinary team, planning and delivering OT services for seniors in home and community settings. You will assess functional abilities, develop treatment plans, and educate participants and caregivers on ADLs and equipment.

The role emphasizes collaborative care, documentation accuracy, and ongoing professional growth within a supportive, mission-driven program focused on dignified aging in place.

Qualifications

• Advanced degree in OT from an accredited program.
• Current state license to practice Occupational Therapy.
• Experience on an interdisciplinary team preferred; training provided if needed.

Responsibilities

• Assess participants' functional abilities and home environment.
• Plan, implement, and revise individualized OT treatment plans.
• Deliver OT interventions across settings (center, home, contracted facilities).
• Educate participants and caregivers on equipment use and ADLs.
• Maintain timely documentation per standards and regulations.
• Participate in daily IDT meetings and care coordination.
• Supervise COTAs and mentor staff on OT practices.
• Train staff in safe OT techniques and equipment use.
• Support quality improvement and compliance initiatives.
• Maintain licensure and ongoing professional development.
