Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role evaluates residents, develops individualized treatment plans, and delivers evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You’ll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• OT licensure in applicable state.
• Strong communication with residents and families.
• Ability to work with interdisciplinary team in SNF/post-acute settings.

Responsibilities

• Evaluate residents and determine needs for ADLs, mobility, cognition.
• Develop individualized treatment plans and document progress.
• Deliver evidence-based interventions to improve safety and function.
• Document in EMR and track outcomes over time.
• Educate families and caregivers on home care and safety.
