Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role involves evaluating residents, developing individualized treatment plans, and delivering interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Licensed OT with IL license or eligibility.
• Experience in skilled nursing/post-acute settings preferred.
• Ability to develop and implement individualized treatment plans.

Responsibilities

• Evaluate residents and plan individualized OT treatments.
• Deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.
• Document progress in EMR and communicate with families and caregivers.
