Seeking a Nurse Practitioner to provide effective and equitable value-based primary care to Medicare patients in Reading, PA.

Opportunity Details:

1. Bilingual Spanish Preferred
2. Full-time, Mon - Fri, 8am-5pm
3. Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate required
4. National certification as a Family, Adult or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC
5. Experience in primary care: Internal medicine, geriatrics, or family medicine preferred
6. Understands HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) Coding, and Health Risk Assessments (HRAs).
