Job summary

Geriatric Medicine Associates is seeking a Geriatric Nurse Practitioner to join our outpatient clinic serving the Denver metro area, split between Lakewood and Westminster. The position offers a competitive salary and benefits with no call obligations and Epic EMR.

You will perform comprehensive health assessments, order and analyze tests, coordinate follow-ups, and educate patients and families while collaborating with the healthcare team.

Qualifications

• Must hold current, valid Advanced Practice Nurse licensure from the Colorado Board of Nursing/Colorado Medical Board.
• Prescriptive Authority valid in Colorado.
• DEA registration/certification current.

Responsibilities

• Performs comprehensive patient health assessments, including health history and annual wellness visits.
• Orders, administers, and analyzes diagnostic tests.
• Performs advanced technical procedures as needed.
• Establishes follow-up to evaluate the plan of care and initiates referrals as appropriate.
• Maintains accurate, confidential medical records.
• Trains patients and families on illness management and prevention.
• Develops strong working relationships with the healthcare team.

The Practice:

Independent group practice of 13 physicians and one geriatric nurse practitioner seeks a geriatric nurse practitioner to join our established outpatient geriatric clinic. The positionwill split the practice days between our Lakewoodand Westminster offices. This position offers competitive salary and benefits. No call obligations. Epic EMR. Great work environment.

The Position:

• Performs a comprehensive patient health assessment, including health history, annual wellness visits
• Orders, administers and analyzes diagnostic tests.
• Performs advanced technical procedures.
• Establishes systematic follow-up for evaluation of the plan of care and reassesses and modifies the plan, as necessary, including initiation of referrals and consultation with physicians and other health care providers as appropriate.
• Maintains accurate, legible and confidential records.
• Trains patients and families on managing illnesses or injuries and disease prevention.
• Develops and maintains a good working relationship with the healthcare team.

Qualifications (Education/Experience/Knowledge/Skills/Abilities):

• Possess current, valid licensure as an Advanced Practice Nurse from the Colorado Board of Nursing from the Colorado Medical Board
• Valid Prescriptive Authority in the State of Colorado
• Current Drug Enforcement Administration (DEA) certification
