Job summary

Vitae Health Systems in Evanston, IL, seeks a dedicated Nurse Practitioner to deliver high-quality, patient-centered care to skilled nursing residents. The NP will assess, diagnose, and manage acute and chronic conditions while collaborating with an interdisciplinary team to improve outcomes and quality of life.

You will provide medical management, order diagnostics, and participate in care plan meetings and transitions of care, ensuring compliant documentation and value-based care delivery.

Qualifications

• Master’s degree in Nursing from an accredited Nurse Practitioner program.
• Active and unrestricted APN license in state.
• Current DEA registration and prescriptive authority (or eligibility).
• Board certification as a Nurse Practitioner (e.g., FNP, AGNP).
• 2 years of clinical experience preferred, ideally in geriatrics or long-term care.
• Strong clinical assessment, diagnostic, and treatment planning skills.
• Ability to deliver high-quality, value-based care focused on outcomes and cost-efficiency.

Responsibilities

• Perform comprehensive health assessments, diagnose and manage acute and chronic conditions, and develop individualized, evidence-based care plans for residents in a skilled nursing setting.
• Provide ongoing medical management including ordering and interpreting diagnostics, prescribing treatments, conducting medication reviews, and monitoring conditions to prevent complications.
• Deliver timely clinical interventions in response to changes in patient status and ensure high-quality, patient-centered care.
• Collaborate closely with physicians and interdisciplinary team members to ensure coordinated, efficient care and participate in care plan meetings and quality improvement.
• Facilitate smooth transitions of care across settings, ensuring continuity and reducing gaps in treatment.
• Educate residents and families on diagnoses, treatment plans, medications, and preventative care strategies.
