Qualifications

Active Nurse Practitioner license in Kentucky.
Experience in a Skilled Nursing Facility (SNF) or geriatric setting preferred.
Strong clinical assessment and decision-making skills.
Ability to work independently while collaborating with a multidisciplinary team.
Excellent communication and interpersonal skills.

Responsibilities

Conduct comprehensive patient assessments and develop individualized treatment plans.
Diagnose and manage acute and chronic conditions in coordination with physicians.
Educate patients, families, and facility staff on disease management and preventive care.
Maintain accurate and timely documentation in compliance with regulatory guidelines.
Collaborate with interdisciplinary teams to ensure quality patient outcomes.
