Job summary

St. Peters Health Partners in Troy, NY, seeks a full-time Nurse Practitioner to join Eddy Heritage House Nursing and Rehabilitation Center, serving 80 long-term residents with geriatric-focused care.

You will work Monday–Friday, five 8-hour shifts, with a manageable caseload of 12-14 residents per shift, including routine and sick visits. On-call is minimal, with one week every 6–8 weeks, and a strong interdisciplinary on-site team supports your practice.

Qualifications

• Passionate about geriatric care and team-oriented environment.
• Board Certification as a Nurse Practitioner.
• Active NYS License and DEA.
• Experience working with geriatric population.
• Geriatrics certification is a plus, but not required.
• Strong communication skills and a commitment to holistic, compassionate care.

Responsibilities

• Provide geriatric NP care to nursing home residents.
• Manage a caseload of 12-14 residents per shift, including routine and sick visits.
• Collaborate with interdisciplinary on-site team (Pulmonary, Physiatry, Psych, Podiatry, WOCN, etc.).
