Job summary
HealthOp Solutions is seeking a Family Nurse Practitioner / Adult-Gerontology NP to lead primary geriatric care across Lake Havasu City and surrounding communities. The position offers a base of $110,000 to $150,000 per year with strong upside potential, plus onboarding and mentorship.
Qualifications
- Licensed Nurse Practitioner with FNP or AGNP preparation.
- Active Arizona NP license or eligibility to obtain.
- DEA registration or ability to obtain.
Responsibilities
- Provide comprehensive primary care to older adults in memory care, assisted living, and independent living communities.
- Manage chronic diseases, acute concerns, cognitive changes, and geriatric conditions.
- Develop care plans focused on health, function, independence, and quality of life.
- Coordinate with families, facility staff, and physicians to optimize patient care.
- Maintain accurate, timely documentation and participate in on-call coverage.
