TruHealth, part of American Health Partners, seeks an advanced practice provider (NP or PA) with geriatrics focus to improve outcomes for long-term care residents. You will work with a team of providers and case managers in nursing homes or assisted living communities enrolled in Medicare Advantage plans.

• Masters or better in a related field.
• APRN/PA licensure with geriatrics focus.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• EHR experience and basic MS Office skills.

Responsabilidades

• Assess patients’ medical and mental health needs and plan proactive care.
• Ensure compliance with local, state, and federal agencies.
• Prescribe medications and order lab work and diagnostics.
• Monitor treatment and adjust plans as needed.
• Collaborate with RN case managers as part of the care team.
