Job summary

Ascension Living PACE Michigan in Flint, MI is seeking a Nurse Practitioner to join an established outpatient geriatric care team. This full-time role offers a Monday through Friday schedule (8:00–4:30) with a 1:5 call structure and a focus on longitudinal patient relationships.

You will work within a nonprofit health system, collaborating with physicians and APPs to deliver holistic care, manage a patient panel, and participate in CME and professional development benefits.

Qualifications

• BLS Provider within 30 days of hire date.
• Nurse Practitioner licensure required.
• Registered Nurse licensure required.

Responsibilities

• Outpatient geriatric care for a patient panel.
• Collaborate with interdisciplinary team to deliver holistic care.

• At Ascension Living PACE Michigan, we believe that geriatric care should be deeply relational, not transactional. Located in the heart of Flint, our team is comprised of a tight-knit, collaborative group of Physicians and APPs who prioritize a culture where every voice is heard and valued. We offer a truly sustainable practice model: with a manageable caseload of approximately 80 patients and a focused daily volume of 2 to 6 visits, you are afforded the meaningful time required to build genuine, longitudinal relationships with your patients.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Nurse Practitioner obtained prior to hire date or job transfer date required.
• Registered Nurse obtained prior to hire date or job transfer date required.

Education

• Master's degree of Nursing required.
