Autumn Lake Healthcare at Glade Valley in Walkersville, MD is seeking a compassionate Geriatric Nurse Assistant (GNA) to provide high‑quality care in a sub‑acute and long‑term setting. You will assist with bathing, dressing, grooming, mobility and transfers; monitor vital signs; maintain accurate records; and collaborate with the healthcare team to implement individualized care plans.

Certified GNA credentials preferred.

Our state-of-the-art facility provides exceptional sub-acute rehabilitation and long-term care services, with a focus on comfort, dignity, and well-being for our residents.

Responsibilities:

• Provide direct care to our residents, including bathing, dressing, grooming, and toileting
• Assist with mobility and transfers, as needed
• Monitor vital signs and report changes to the healthcare team
• Maintain accurate records and reports
• Collaborate with the healthcare team to develop and implement individualized care plans
