Job summary

CareMed is seeking a Nurse Practitioner to provide primary and preventive care for residents in skilled nursing facilities in Iowa. The role includes managing acute changes, advanced care planning, and chronic disease management in an interdisciplinary team.

Candidates must hold an unrestricted Iowa NP license, board certification, and DEA licensure, with experience in long-term care and EMR systems. The position offers full-time hours with comprehensive benefits.

Qualifications

Current, unrestricted NP license in the state of Iowa.
Board Certification.
DEA licensure.
Clinical background in adult, family or geriatric specialties.
Experience working in a long-term care setting.
Ability to work with an EMR.
Ability to manage patients with advanced illness and multiple chronic conditions.
Ability to work within an interdisciplinary care team to coordinate patient care.
Psych experience a plus

Responsibilities

Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
Manage Residents with acute change in condition.
Advanced care planning and chronic care management.
Prescribe appropriate medications, including controlled substances in accordance with Iowa Law.
Order, review and/or follow-up with treatments and diagnostic testing.
Communicate with the nursing home staff on a regular and as needed to discuss changes in condition and/or the care plan.
Collaborate with primary care physician.
