Job summary

Novant Health Community Health & Wellness Institute seeks an experienced Advanced Practice Provider (PA or NP) to join our geriatric LTC rounding team across Skilled Nursing Facilities in the Triad, NC region.

The role offers autonomy in LTC settings and requires a Master’s degree with proper licensure, NCCPA or NP credentials, and a commitment to geriatric care. You will work with LTC staff to provide high‑quality care across multiple facilities.

Qualifications

• Master’s degree in PA or NP; NCCPA accredited program or NP pathway required.
• Current PA or NP license in appropriate state; NCCPA or registry with state nursing board, BLS, and DEA verification required.
• Previous long-term care or geriatric care experience is preferred.

Responsibilities

• Round in multiple skilled nursing facilities and LTC settings.
• Provide autonomous clinical care within OP/ inpatient LTC teams.
• Collaborate with LTC staff and other providers to ensure continuity of care.
