Job summary

OptumCare is seeking a Nurse Practitioner or Physician Assistant to provide daily rounding for patients in skilled nursing facilities and long term care settings. You will serve as the primary provider for 1–2 SNFs, managing patients across skilled and long term care levels, with 6–9 encounters daily depending on complexity and facility count.

You will participate in weekly interdisciplinary team meetings and collaborate closely with Extended Care physicians and the patient\'s primary care team

Qualifications

• Current Washington state license
• NP-C or PA-C certification or ability to obtain before hire
• Strong communication skills and ability to promote teamwork

Responsibilities

• Provide daily rounding on patients in SNF and LTC settings
• Serve as primary provider for 1–2 SNFs
• Complete 6–9 patient encounters daily depending on case mix
• Participate in weekly interdisciplinary team meetings and other departmental meetings
• Collaborate with Extended Care physicians and primary care team to deliver high quality, value-based care
