Job summary

Community LIFE is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) for primary care in Somerset, PA. This full-time role supports frail elderly participants with comprehensive medical care and coordination across the Interdisciplinary Team.

The position requires NP/PA education, PA license with ARC-PA certification, and PA driver’s license. Shift is Mon-Fri, 7:30am-4:00pm; excellent benefits offered.

Qualifications

• Completion of an accredited NP/PA program.
• PA license with ARC-PA certification.
• Prescriptive Authority and DEA license.
• Current PA driver’s license.
• 1 year serving frail elderly; 3 years NP/PA preferred.

Responsibilities

• Assess, monitor, and manage medical conditions of participants.
• Oversee use of medical specialists and inpatient care.
• Collaborate with Interdisciplinary Team to plan and deliver care.
