Qualifications

• Nurse Practitioner certification required
• Minimum of two years of Nurse Practitioner experience
• Active New Mexico Nurse Practitioner license, or ability to obtain
• Passion for caring for older adults and delivering patient-centered care

Responsibilities

• Providing comprehensive outpatient care for older adults
• Conducting annual wellness visits and preventive health screenings
• Diagnosing and treating acute and chronic conditions
• Managing geriatric concerns including dementia, osteoarthritis, incontinence, and multiple chronic conditions
• Ordering and interpreting diagnostic tests and laboratory studies
• Developing and managing individualized treatment plans
• Collaborating with physicians, specialists, nurses, caregivers and interdisciplinary team
• Supporting patient goals while promoting independence and safety
