Job summary

Qualifications
- Active NP certification focused on geriatrics.
- Minimum 2 years of geriatric clinical experience.
- Strong clinical skills in geriatric assessment, diagnosis, and treatment.
- Excellent communication and collaboration with interdisciplinary teams.
- Compassionate, patient-centered approach and proficient EHR use.
Responsibilities
- Conduct comprehensive physical examinations for geriatric residents.
- Develop and implement individualized care plans.
- Prescribe medications and order/interpret diagnostic tests.
- Collaborate with physicians, nurses, social workers, and therapists.
- Educate residents and families about health conditions and options.
- Advocate for residents’ well-being and appropriate care.
- Document patient care thoroughly in the EHR.
