Metropolitan Jewish Health System, Inc. (MJHS) is seeking a Nurse Practitioner to coordinate primary care for Advantage members residing in long‑term care facilities and living at home.

You will collaborate with physicians to ensure preventive care and optimal outcomes within a not-for-profit health system in the Greater New York area. The role requires an NP with geriatric experience, NYS RN/NP license, and a master’s degree in nursing.

Qualifications

• Coordinate primary care services for Advantage members in long-term care facilities.
• Develop and access preventive care plans aligned to member goals.
• Clinical resource for participating providers and facilities.

Responsibilities

• Develop care plans and coordinate primary care for assigned Advantage members in long-term care.
• Provide and coordinate primary clinical care in nursing home residents.
• Serve as a clinical resource for providers and facilities and a point of contact for care plans.
