Job summary

Metropolitan Jewish Health System (MJHS) is recruiting a Nurse Practitioner to provide and coordinate primary care for Advantage members in long-term care facilities and home settings. You will collaborate with physicians to deliver comprehensive geriatric care and manage clinical needs for an assigned panel.

Qualifications

• Masters degree in nursing from an accredited program.
• Active NYS RN/NP license and registration.
• DEA certificate preferred.

Responsibilities

• Coordinate primary care for Advantage members in long-term care facilities.
• Develop and manage care plans to meet preventive and chronic care goals.
• Serve as a clinical resource to participating providers and facilities.
• Act as a liaison for member and designated representatives regarding care plans.
