Job summary

Frey Consulting Group is seeking a Family Nurse Practitioner or Adult-Gerontology NP to lead geriatric primary care across Lake Havasu City and surrounding areas. You will provide comprehensive care in assisted living, memory care, and independent living communities, building lasting relationships with patients, families, and facility staff.

The role emphasizes strong onboarding, mentorship, and ongoing coaching, with a clear path to leadership, education, research, and university collaboration

Qualifications

• Licensed Nurse Practitioner with FNP or AGNP preparation.
• Current Arizona license or eligibility to obtain Arizona licensure.
• DEA registration or ability to obtain.
• Strong clinical decision-making and critical-thinking skills.
• Excellent interpersonal and communication skills.
• Thorough documentation and time-management habits.

Responsibilities

• Provide comprehensive primary care to older adults in memory care, assisted living, and independent living communities.
• Evaluate and manage chronic diseases, acute concerns, cognitive changes, and medically complex geriatric conditions.
• Conduct preventive care, routine assessments, and follow-up visits.
• Develop individualized care plans focused on health, function, independence, dignity, and quality of life.
• Manage medications and reduce unnecessary polypharmacy when clinically appropriate.
• Coordinate care across senior living communities, hospitals, and specialists.
• Educate patients and families regarding diagnoses, treatment plans, and goals of care.
• Maintain accurate, timely documentation and support proper billing and compliance.
