Job summary

South River Healthcare Center in Edgewater, MD, is seeking a Nursing Practitioner (Adult/ Gerontology) to provide high-quality primary care in a high-touch, person-centered model. The role involves managing acute and chronic conditions, collaborating with the PCP and interprofessional team, and supporting value-based care initiatives.

The position offers a $15k sign-on bonus for a 1-year commitment, work Monday–Friday, with no on-call, weekends, or holidays.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• National certification of AANP or ANCC.
• Active, unrestricted state license.
• Active DEA license.
• US work authorization.
• 2+ years clinical experience as a Nurse or Nurse Practitioner in geriatric, adult or family practice setting.

Responsibilities

• Assess, diagnose, and manage chronic and acute conditions.
• Provide health education.
• Develop treatment plans and communicate them to residents and providers.
• Order and interpret lab and diagnostic testing and adjust plans accordingly.
• Maintain EMR documentation.
• Collaborate with local and regional operations and clinical leadership to achieve goals.
• Support value-based care initiatives.
• Provide excellent customer experience and retention.
