Job summary

RAM Recruiting is seeking an experienced Nurse Practitioner to provide primary care services to residents in an assisted living setting. This role offers autonomy, predictable scheduling, and the opportunity to build real continuity of care without nights or hospital rounds.

You will conduct routine visits, manage chronic conditions, and collaborate with interdisciplinary teams. New York NP license and DEA registration are required, with 3+ years geriatric or primary care experience preferred.

Qualifications

• Master's degree from an accredited Nurse Practitioner program.
• Active New York State NP license.
• DEA registration.
• Eligible for Medicare and Medicaid billing.
• Minimum of 3 years clinical experience preferred.
• Geriatric, long-term care, or primary care experience strongly preferred.
• Strong clinical judgment and patient-centered approach.

Responsibilities

• Conduct routine and follow-up visits for assisted living residents
• Perform comprehensive assessments, medication reviews, and exams
• Manage chronic and acute conditions
• Develop and update care plans in collaboration with interdisciplinary teams
• Order and interpret labs and diagnostic tests
• Prescribe medications within scope of practice
• Educate residents and families on health management and prevention
• Maintain accurate documentation in eClinicalWorks
• Participate in quality and clinical improvement initiatives
